Safety On Site (SOS) – Safety File Information Form
Please complete this form as accurately and fully as possible. The information you provide will be used to generate your Health and Safety File, HIRA, Method Statements, appointments, registers, and supporting safety documentation.
To create the correct file for your work, SOS requires full and accurate information about your company, the site, and the exact work to be carried out.
Please do not give only a short description such as “electrical work”, “maintenance”, or “roof repairs”.
You must provide the full scope of work and include everything that will be done on site.
Your scope of work must clearly describe:
the exact work to be carried out
how the work will be done
where the work will be done
what tools, plant, equipment, machinery, access equipment, and materials will be used
whether there will be work at height, electrical work, lifting, chemicals, hot work, excavation, confined spaces, public exposure, or any other high-risk activity
the number of workers involved
any site restrictions, access limitations, emergency concerns, or client requirements
